2016 Medical Action Plans

It is a requirement of De La Salle College enrolment that any student identified as having a medical condition requiring an Action Management Plan must provide a 2016 plan, reviewed and signed by a medical practitioner.

Relevant families will receive a letter outlining their son’s medical condition currently on record at the Health Centre and will be required to download the applicable Action Plan (listed below) and submit to the College prior to the commencement of Term 1, 2016.

ANAPHYLAXIS

If your son suffers from anaphylaxis you are required to complete a current ASCIA Action Plan — Anaphylaxis.

ASTHMA

If your son suffers from asthma you are required to complete a current Asthma Care Plan.

ALLERGIC REACTIONS

If your son suffers from allergic reactions (including hayfever) you are required to complete a current ASCIA Action Plan for Allergic Reactions

DIABETES

If your son suffers from diabetes you are required to provide a current Diabetes Management Plan from your Diabetes Educator or Doctor.

EPILEPSY

If your son suffers from epilepsy you are required to complete a current Epilpesy Management Plan AND a DLS Student Medical Condition Management Plan.

OTHER MEDICAL CONDITIONS

If your son suffers from any other medical condition (not outlined above e.g. heart conditions, migraines etc) you are required to provide a current DLS Student Medical Condition Management Plan.

Please download and complete the applicable action plan/s and return completed documents to the College by Wednesday 27 January 2016 via email – dlsadmin@delasalle.vic.edu.au or post.
