From the Health Centre
At the beginning of the school year, the college sent out an email requesting relevant paperwork regarding your son’s medical conditions and concerns. The Health Centre is in the process of updating the information from Consent 2 Go (C2G).

If you have identified your son suffers from a medical condition, an action plan is required. The action plans and medical authority forms are required to be signed by a GP with their signature, AHPRA registered number and Clinic stamp on same for it to be legally accepted.

You may be contacted by your son’s Health Centre/Year Level or House Coordinator requesting the correct medical paperwork over the coming weeks as the new information is processed. Failure to provide the correct paperwork may result in this matter being escalated to your son’s Director of Students or Head of Campus and may prevent your son attending camps/excursions until the correct paperwork is received.
